Al says that 60% of the time we can make a very small profit, or a very small loss. 40% of the time we can make a big profit.
If we are supposed to have all trades be the same exact loss when stop loss is hit (i.e., 20 USD), how can we have a very small loss when MTR does not work out and stop loss is hit?

Because the stop is not hit, you exit before when it is clear that the trade is not working as expected.

This is one method. I have also seen multiple situations, though, where there is no chance to exit the position.
